If you change one thing on your Google Business Profile this month, change the primary category. It is the single field that decides which searches your business is eligible to appear for, and it is wrong on roughly half the UK profiles we audit — not because owners are careless, but because the obvious choice is often not the one Google rewards.
Google's local algorithm starts by asking "which businesses could be relevant to this search?" The answer is drawn overwhelmingly from categories. A search for "dentist" returns profiles whose primary or secondary categories are dental; a "Medical clinic" with three dentists on staff does not qualify, however good its reviews. The primary category carries by far the most weight — it is shown under your name, it drives which searches you appear for, and it determines which attributes and features (menus, bookings, services) your profile is offered.
Secondary categories extend eligibility to related searches at reduced weight. A profile with primary "Dental clinic" and secondary "Orthodontist" can appear for orthodontist searches, usually below profiles whose primary is "Orthodontist".
Not from a list. From the pack.
The result is often more specific than instinct suggests. "Roofing contractor" rather than "Contractor". "Emergency plumber" rather than "Plumber" in areas where emergency work dominates. "Cosmetic dentist" rather than "Dentist" for a practice that lives on veneers and whitening. Google has over 4,000 categories and adds more every quarter; the specific one usually has a smaller, easier pack.
Add a secondary for each distinct thing you do that has its own category: an accountant adds "Tax preparation service", "Bookkeeping service", "Payroll service". A garage adds "MOT testing service", "Tyre shop", "Car battery store". Up to nine secondaries are allowed; most businesses have three to six that are genuinely true.
Do not add categories you do not operate. "Plumber" on an electrician's profile because they occasionally fit a shower is not true, dilutes relevance for electrical searches, and is the kind of thing Google's eligibility checks pick up. Categories are also visible to competitors, who report them.

These are starting points. The pack in your area decides.
A business that genuinely straddles categories — a clinic with dentists and physios, a shop with a café — should pick the primary that earns the most and cover the rest with secondaries. Splitting into two profiles at one address is only legitimate when they are genuinely separate businesses with separate names, entrances and phone numbers. Practitioner profiles (an individual dentist or solicitor) can carry a different primary from the practice, which is useful, but should not duplicate the practice's primary or they compete with it.
Google re-evaluates the profile within days. If the change is significant you may be asked to re-verify, so have your evidence ready. Ranking movement for the new category's searches typically shows within two to four weeks; watch it on a grid, not from your desk. Movement for the old category's searches may go the other way — which is the point.
